Why are you letting your engine warm up? Start it and drive. How long does the engine have to sit before it exhibits these symptoms? What happens if you put it in gear for say 15 seconds before you try to pull away?

You could try seeing that it has adequate fluid, and change it as well. Pray for that to remedy the problem. Sounds like a a centrifugal clutch problem. Any trouble when it changes gear in motion?
