I found somebody who still has the adapter!!!

Downtown Gas & Auto, 570 Eastern Ave, 416 463 1138. Careful job reasonably priced.

FWIW, they appeared to use an 10-lug one ton truck adapter (poss. GM) and just used five of the holes.

You can forget external balancing by puttin 3-4 OZ of balancing beeds insde the tire. One time and forget it.
