I would go and check all the basics:

1.) Do the temperature sensors (coolant and IAT) measure correctly? Verify with VCDS.
2.) Is the timing set right? The correct procedure is a real pain. Make sure you've done it right. There are reports of this symptom with ACU engined vans when the timing isn't set right.
3.) Is the idle air control solenoid clean? Sometimes they get gunky and stick, causing this sort of thing.
4.) Has anyone messed with the basic idle screw on the throttle body? It should never need adjustment, and should be sealed with a paint dot, but you never know.
5.) Are there any intake leaks?
