Anyone know how to properly fill this out? Indiana’s very picky and I don’t want to screw it up. I’m the buyer and not a dealer.

Looks to me you fill in section 3.
Call your local Motor vehicle department, ask them. It seems you fill out a title transfer form, and will need to have old title in hand for the clerk to check.
They might want you to surrender the old title to get a new one. If you don't like that, try telling them the old title belongs to you and the state of Wisconsin, not Indiana. Make a color copy first as they may say too bad.
You need to fill in the sellers name. I had a hell of a time finding the right color pen to do so once. Get a lefty to fill in name so it looks nothing like your handwriting. |

Since you're looking to title and register in a different state, most likely all you need to do is finish filling out section 1 with your name (purchaser) and the date. I'm guessing the slip of paper is covering the seller's signature, if not, that will also be required. The DMV will have you fill out the appropriate title application form which covers pretty much everything that section 3 of the title is looking for -- worst case scenario you'll have to fill it out as well, but there's nothing in there that requires the seller to fill in or sign. |

go in to the RMV/DMV and have them tell you how to fill it out if you can. |

Thanks, section 1 was all I needed to fill out, got the new Indiana title today. It’s for my ‘59 Impala. I would’ve posted this question in a Impala forum but none of them are as active as the Samba. |
