I’m going to be honest here... I have one. I didn’t use it because I thought the chunky #2 spark plug reading was proof enough. But it wasn’t, and I’m seeking opinions on how #3’s plug was so clean while the cylinder was caked in carbon.
It was only an extra hour and change to do the second head; thank god the engine only has like 10k on it so nothing was stuck, rusted, or out of spec.*

That’s very common. The mechanic has a solid hunch that the cause must be such and such. Later finding his assumption to be wrong. It’s happened to me numerous times. I find that usually we assume that the cause must be the hardest, most expensive thing, like a bad ecu only to find later that it was just burned points or something else like a bad spark plug ( happened to me once).
Tools like boroscopes are great for diagnostics. All we need to do Is think about taking the 15 minutes needed to use it. _________________ Lead Mechanic: San Diego Air and Space Museum

Well, that's interesting. Given the W I D E gap allowing the shrapnel to descend into the sump, I wonder...
1) How common is this problem?
2) How long will it be before it happens again? |

Glad you got it all sorted!
So we're saying the green side of the broken expander is still in the sump?
What did the top of #2 look like, with the bad plug? Might have weak spark on that cyl or something. (?) Did you replace plug 2?
Did you change all 4 expanders? Review the other 3 expanders for cracks?
Things to keep an eye on.

Your sockets are out of order and one is upside down
The green part of that ring will be on the right side of the engine, the red, to the left
