Has anyone used the "ITP 12mm Flat Base Lug Nuts" for their vintage wheels like crestlines or calparts? I know they are for ATV's but wondering about their strength on a car.

That's not where they are made to take the load, I would see round lug holes not being so round after a while with that arrangement. Saftey could be an issue too.

I machined down straight shank lugnuts to fit my Crestlines.

Heh hows it going. What diameter studs do you have? Are crestlines like calparts at the lug area?
My 12mm stud fits super tight through the wheel. Have you got a picture of your set up.
