'91 Slider door falling off :(

So our old faithfull '91 Vanagon has a Slider door issue, it falls off... Sad

So after reading the Bentley & various threads here on The Samba, I've come think it is my Bearing Roller Assembly wheel thingy:

The door opens fine, slides along the track & drops off about half way open,
So my questions are : Is that roller wheel all worn off on the inside rim- (pic w/ my finger in it) or does it only have a tall edge on the outer side of the roller ?? W/the door closed that arm pivots out - away from the track, is it supposed too ??

There are no holes or rot in any of the tracks, this seemed to start when the back edge of the interior panel popped out, and got hung up on the body, I did'nt know why it wouldn't open, pulled harder from the outside to open the door, it slide half way open & the door dropped. Could that interior panel pushing the door away from the track cause it to jump off the track ?? Or bend the track out a little ?? Or is that wheel worn off on the inside edge ??
The Bently shows the wheel to look more like a spool or rim, w/ a tall edge on each side.

Any Help ??
