Very rare shirt with a dualport head. Art work done by Jim Phillips.
I had a chance to buy one of these shirts at a Levi's store back in the late 90's for $80 or so. I have been looking for a photo of this shirt online, on & off for years. 
I finally found it a few days ago on ebay in Europe.

I've always been a fan of Jim Phillips and his son Jimbo Phillips Artwork.
A cross between Salvador Dali & Ed "Big Daddy" Roth.
Here's a Patch with the same VW artwork ... Volks Machine Works Patch.

I contacted Jim Phillips via email at [email protected] and he responded that the artwork was done in the 70's for Volkswagen Machine Works in San Luis, Obispo, CA. Licensed to LEVI'S twice 2012 t-shirts in FRANCE and again in 2015 for USA which included a "trucker" style LEVI'S Denim Jacket.

After researching this Logo I purchased this Volks Machine Works, Levi's Jersey T-Shirt.

THAT IS AWESOME!
I'm a HUGE Phillips fan-- I'm wearing Screaming Hand socks right now! I've seen lots of his non-skateboarding related work, but never this one. THIS IS SO COOL!!1

I was lucky enough to find the t-shirt in a store; one of my favorite!
It comes with a sticker pack; here are some pics (there was also the famous Screaming hand; actually missing because I used it on my Suzuki RV90  )
