How does this sort of wear happen? maybe loose bolts, or did someone beat it with a hammer for some reason.

Do you think this is OK to reuse?

Looks like someone beat it with something. I'd reuse it as is. You could use a welder to build the area back up.

Someone used a sledge to remove it from a center section at some point. Must have had a sticky bearing. Or more likely they had not removed a part needed to take the box off. Laughing

Tap bolt hole threads to make sure the bolts spin in by hand.

Small nut RGBs are a PITA to disassemble. Some guys think a BFH is the answer...... Rolling Eyes

If you fill that pocket on the inner side of the RGB with even old used grease, before bolting to the swing plate. It will keep mud and water from collecting in that area and causing lots of rust there and in the threads of the bolts. Of course do use an anti-seize on the bolts also.

I added some material and filed it level. Should be good as new

Looks good. Run a tap through those threads or a wire brush on a drill.

Good to drill/tap them deeper so you can use 40% longer bolts. That way you can skip having the old tired RGB threads stripping:
