Anyone have a picture of an example of this birth cert desc?

Hi there everyone,

I was wondering if anyone might have or know of a picture that would depict this birth certificate description.

My friend bought me a birth certificate and although Zippy won't be returning to his birthday build, I'd love to see how he rolled off the assembly line.

I added the rest of the certificate just because it would be cool if anyone were looking for him. Any additional historical anecdotes based off of the info would be most welcome as well.
