When I open the fresh air vents on the dash they only push hot air instead of cool outside air. The heat is not turned on when this happens. Has anyone seen this on their van? Any idea how this system is assembled?

Perhaps not but after your next drive, pull off the lower console/shroud and feel the two hoses that go to the heater core. If they are hot, it's likely the control valve that VW cleverly installed below the cab behind the fan has given up. You can either R&R it or add a more accessible one in the cabin.

The heater temperature lever moves a cable that opens and closes a valve in the heater hose. More or less hot coolant flows through the heater core depending on the position of the lever. The valve may not be fully closing anymore. Sometimes the cable is the problem, other times the valve itself has a problem. It is cheap to fix either way.

What year is your Vanagon? The dash vents differ based on model year.

Thanks for the troubleshooting tips. The vehicle is actually a 1987 syncro Doka from Sweden so it may have a unique heating set up.

I was able to use the Ford valve inside the cabin and jury rig the existing cable to operate it. I left the stock valve open in place.

I thought the fresh air vents (that can be shut off) were meant to be fresh air only, from before the heater core.

That changed early in the 87 model year. After that the end vents only get air that has passed through the core so a heater valve that doesn't fully close is an even larger problem. I have 4 of these 87 vans and the valves work fine in all of them. I have replaced the valve in all of them over the years as they don't last forever.

Before installing a new valve, close it and try to blow through it. If it leaks any air at all, get one that doesn't leak air.

VW Parts says only the first 13,413 of the 1987 models still had the old fresh air only to the end vents.
So about 90% of 87s got the later heated end vents.
