Rear axle nut loosened. Not sure why.

So, before the sarcastic remarks start, yes, it was torqued to spec.
Now for history:
This happened once before. I chalked if off the first time to "possibly" not getting it tight (altho, I think I did). That time, it "wore" off the spines on the rotor completely and when I took it apart, there were NO pieces of splines to be found. (maybe 6 years ago.)
This time, I felt/heard something in the brakes and thought I maybe had a warped rotor. I pulled the drivers side rear wheel off and was looking at the rotor and it felt ever so slightly loose. I grabbed the nut and sure enough....lose. The hair pin was still intact. I actually unscrewed it by hand, no wrench at all.
So, is there possibly something I am doing wrong, or have messed up, that allows this? Like some wear occurring someplace else??
I definitely want to get it right as BedbugI want to take it to the Smokies in a couple weeks to ride around and enjoy the mountain roads. (SOOOO fun!)
Also, I'm planning to "drive it" to Key West in the Fall, from Indiana. (I already have East Coast to West Coast, via US-50 under my belt so I need the southern most point now.)
BTW, the rotor might have a couple thousandths of play on the stub but, unless it NEEDS zero play, I think it is fine. It is just barely noticeable.

Typical procedure is to go to 265 ft-lbs and continue on till you get a set of axle holes to line up with castle nut.... Hair pins in my auto-x group were considered cool, but I stayed with cotter pins....

Empi rear disc brakes? I had a similar experience. Empi was kind enough to replace them.

Makes me wonder: Could the rotor's hub actually get sorta squished from 265+ foot-pounds of torque; I mean, what kinda metal are they made of anyway? (My Chinese rear drums are made of some awfully soft metal.)

Did you use the early style axle nuts or the later version with a flange built in? The later one work better. Tom

When you removed the hub, was the small o ring on the axle all chewed up?

I had one car where the axle nut like to loosen up. When I pulled the bearing I found the inner spacer installed the wrong direction. After replacing the inner bearing spacer and installing it the right way (large radius against the seat on the axle) the problem went away. This was with stock drum brakes on a '63 Bug.

Ha ha, and I've also heard of engine and trans cases made of "used toothpaste!" But even tho my Chinese rear drums are so soft that I could easily file down their sorta pot-metal for clearence of brake parts, I've had no castle nut loosness.
